Effective pain relief in postoperative patients Essay Pain is physiological mechanism and a means of the body alert the person about either currently present or impending damage to it. It can be defined as the ââ¬Å"unpleasant sensory and emotional experience associated with actual or potential damageâ⬠(Gelinas, 2004). Consequently, the words discomfort has also been used interchangeably with pain in the past and can be defined as a ââ¬Å"negative affective and/or physical state subject to variation in magnitude in response to internal or environmental conditionsâ⬠(Gelinas, 2004). Pain is a very important component in care of the surgical patients, both preoperative and post operative. Surgery itself afflicts large amounts of trauma on the patient. The pain that the patient has to incur afterwards adds his anxiety levels and stresses the body. Lack of or inadequate amounts of pain management by the staff, impacts the patients healing process. Their mobility and in the long terms their duration of hospital stay is also affected. It is important that, for optimal patients health management and care, a means tool must be available to measure the patients pain assessment and relief requirement through pharmacological and non pharmacological means. Little research has been done to properly assess and document the prevalence of pain and its management protocols in post operative patients. The pain scoring systems are available to attempt to quantify pain and manage it accordingly. These subjective ones include the categorical rating scales (CRS) in which patient rates pain from ââ¬Å"Noâ⬠to mild moderate or severe. Another one is the ââ¬Å"Visual Analog score (VAS) where the individual scores are placed on a 10-cm line where the left anchor point is labeled ââ¬Å"no Painâ⬠and the right anchor point is labeled ââ¬Å"worst possible painâ⬠. Since every patient has a varying threshold for pain, and requires varying levels of drugs, to over come the pain, it is better for there to be both a subjective indicator for pain as well as an objective one, that is to say, that the nurses an also assess how much pain the patient is in, by using a tool she is provided with. Cardiac surgery is a major thoracic surgery and patients post operatively require lot analgesics to manage pain so that their morbidities associated with pain (like pain in breathing, walking etc) can be eliminated. Usually morphine is used for the purpose of pain relief but there are indications that Fentanyl can perform a similar relief without much of the side effects associated. There needs to be some focus into this theory. This can be achieved by placing 2 similar populations of adult cardiac patients who have just undergone cardiothoracic surgery on morphine or Fentanyl. As is the standard procedure nowadays, quality indicators, both subjective and objective can be used. The VAS pain scoring card can be provided to the patients for them to record the different levels of pain that they feel. The nurses would be provided with indictors to record objective findings which can indicate pain. ââ¬Å"These can be physiological and behavioral indicators. The physiological indicators can be clustered into cardiovascular, respiratory and cerebral responsesâ⬠(Gelinas, 2004). Pain associated responses would include tachypnea in respiratory, tachycardia and increased blood pressure in cardiac and raised ICP in cerebral responses. After assessment of the pain levels with the quality indicators, the pain management drugs will be administered and later the subjective and objective pain assessment will be repeated to see which drugs effects were greater and lasted longer. A study conducted by Celine Gelinas on critically ill incubated patients, to find out what are the protocols and tools used to assess pain management are and if the patients were being given effective relief. He used subjective as well as objective tools. The results revealed that physicians placed no role in documenting pain in patients. Most of the reporting was done by nurses and the patients. It was also noticed that nurseââ¬â¢s assessment of pain was much less than that reported by the patients themselves. The research concluded that the documentation overall about the pain and its management was incomplete in general with little attention being given. The research also noted that even after being notified about the pain, its effective management only took place 60 percent of the time. (Gelinas, 2004). In and interventional study conducted by Francoise Bardiau in 2003, the quality indicators e. g. VAS were introduced in the surgical and anesthesia department. After a survey of assessment of knowledge of nurses, VAS to assess pain was the nurses worked to improve pain management. After further surveys, it was noted that initiation of programs to setting of quality indicators improves the overall pain management system. (Bardiau, F. , M, 2003) Idvall E tested a 5 point scale to measure the effects of quality indicator maintain pain relief measurements. ââ¬Å"The results suggest initial support for the new instrument as a measure of strategic and clinical quality indicators in postoperative pain management, but it must be further refined, tested and evaluatedâ⬠. Idvall E 2002) a multidisciplinary program development was introduced based on evidence based medicine to focus on construction of proper management protocols to implement clinician as well as patient based pain relief programs ââ¬Å"The results suggest that addressing pain management through a variety of strategies targeted at the level of the institution, the clinician, and the patient may lead to desired changes in practice and better outcomes for patients. .â⬠Bedard, D (2006). A survey conducted on post operative cardiac ICU patients, about their pain experiences revealed that despite the pain management regimes in place nowadays, the pain frequencies, and intensities were the same as they were more than a decade ago. Pain management is a vital component of patient care. Quality of pain management can only be assessed through proper indicators. These can be multimodal. The patient populations on which these indicators can be applied are preoperative and postoperative patients. Post operative cardiac patients under nursing care can benefit well from implementation of quality indicators such as VAS. In the nursing profession subjective scoring by the patients themselves enables the nurses to manage the pain properly. This will lead to quicker recovery by the patients and earlier discharge. In the long term this means leads financial implications on the patients due to reduced hospital stay. Also nursing work load gets reduced as the patient tern over is increased. The healthcare cost gets reduced. A Post operative pain management (POP) project was conducted in 2003. A nationwide survey was done to see the implementation of quality improvement projects in the field of pain management, it was noted that more than 70% of the hospitals were reportedly satisfied with the implementations and the outcomes of the quality improvement programs. Based on the analysis, it is noted that on the positive side, proper implementations of the quality indicators and improvement programs in the healthcare system and especially in the ICU and surgical wards, the patients stay can become quite comfortable. The stay can be reduced and the cost of healthcare to the system, the insurance companies, and the patient themselves can be reduced. In the other hand we can clearly see that by using the indicators we in effect are placing more workload on the nurses. If the subjective VAS and the objective physiological changes in the patient has to be monitored regularly just to assess the pain levels, a lot of quality time will be wasted. This time could have been used to tend to more critical patients. Now the question arises if it is worth the effect to implement the QI programs. The answer would lie in Force field analysis and the Lewinââ¬â¢s theory. If the benefits out way the set backs, we can implement the system. The idealistic thing would be that we assess the pain management needs of each department of the health care system and implement the QIs in the ones in which the implementation benefits out way the costs.

These are the people that are capable in doing the job, not only to fill the vacancies but have contributed well enough to the business, functioned well or even exceeded the job expectations from them. Working experience and educational background helps this people to be more competitive compared to others. This, I termed as the qualified people. They are the effective ones. Business sectors are keeping them. These people are their aces. They are the organizations apple of the eyes. That is why they are hard to find. Few are considered qualified people. Millions of people maybe looking for jobs but are not qualified. It is because these people, considering the poverty growth in the population, they work practically for money. Self-improvement may only be a secondary reason which includes behavior towards work, leadership skills, quality work and the like. On the other hand, qualified people prioritize self-improvement, and, in turn, money comes naturally.

Cheese and wine are also form a major part of the cuisine, serving different purpose both regionally and nationally. Coffee, and more specifically espresso, has become important to the cultural cuisine of Italy. The famous italian dessert Tiramisu is a coffee flavoured dessert. Italian cuisine has evolved extensively over the centuries. This is a very astonoshing fact that the country called Italy today was not officially formed until the 19th century, thus the Italian cuisine can claim its roots going back to 4th century BC. Italian cuisine is a result of various influences throughout the centuries, which include- neighbor regions, conquerors, high-profile chefs, and political upheavals led to the formation of, a concrete cuisine today known as one of the premiere cuisines in the world called the Italian cuisine. The Romans of the early ages did not appreciate high quality ingredients. Thus they employed the best bakers from Greek to produce their breads, imported pecorini from Sicily as the Sicilians were considered as the best cheese makers. They were also known as being excellent rearer of goats for butchering and gardening of artichokes and leeks. In the medieval ages the Muslims invaded Sicily during the 9th century, what is known today as Northern Europe was attacked by Viking raiders. Sicily had already attained a lot of the culinary traditions from the culinary traditions of Rome and Athens, the highly civilized culture combined with the Sicilian cuisine to bring the first real Italian cuisine. The Arabs introduced spinach,almonds and rice. The Romans were considered to be the masters of preservation techniques which were a necessity as refrigeration had not been invented. The two types of preservation were either chemical or physical. During the 16th century the macaroni was cooked in a capon stock which was flavoured with saffron, showing the Arab influence on the Italian cuisine. The early modern era of Italian Cuisine was affected by the courts of Florence, Rome, Venice and Ferrara which were integral factors which led to the creation of the fine cooking in Italy. Italian cuisine took a great diversification in the modern age, what was known as Italy today was governed by France, Spain and Austria during the 18th century. In the beginning of the 18th century that the culinary books of Italy began to show the real regioanalism of Italian cuisine in order for Italian chefs to better show the pride of their regions instead of the high cuisine of France, Spain and Austria . As the century progressed the cuisine increased in size, popularity and frequency, During 18th century peasants lived on a diet which consisted of heavy foods. Taken more as a step toward the high cuisine of the Italy which was more refined and delicate, this diet gave the peasants their own identity.Many peasants had to survive by eating rotten foods and moldy breads to survive. REGIONAL CUISINES OF ITALY. Each region of Italy has their own proud specialties, not primarily at regional level only, but even at provincial level. Old Man and The Sea In the novel The Old Man and the Sea, Ernest Hemingway uses the literary device of metaphors. Hemingway uses the metaphor of the ocean to symbolize life and to depict the role that individuals play in life. Hemingway uses the metaphor of the lions to signify people who live their lives as active participants. The tourists in the novel represent the individuals, who in observe their lives and are not active participants. In the novels that Ernest Hemingway writes, he uses metaphors to reflect his life experiences and opinions. The ocean in The Old Man and the Sea is a metaphor, which represents Hemingway's personel view of life. Hemingway believes that in life everyone must find their own niche and uses the metaphor of the ocean and the boats on it to demonstrate this. ...most of the boats were silent except for the dip of the oars. They spread apart after they were out of the mouth of the harbour and each one headed for the part of the ocean where he hoped to find fish. The old man knew he was going far out...1(page 22) Hemingway feels that in life there are people who participate in life and people who observe life as it passes just like on the ocean where there are boats that do not test their boundaries. The boats are the people in life, and most of the boats are silent. They paddle within the areas they know to be safe and always are cautious not to upset the life that they have established for themselves. Hemingway is explaining that most people don't raise a commotion, they just allow life to happen to them. The old man is testing his limits, he is challenging the ocean, and rowing where he wants to go not where the ocean wants to take him. Hemingway believes that in life, the farther person stays from the observers, the more free and exhilarated they will be. If there is a hurricane, you always see the signs of it in the sky for days ahead, if you are at sea. They do not see it ashore because they do not know what to look for, he thought. The land must make a difference too, in the shape of the clouds. But we have no hurricane coming now.2(page 51) Hemingway theorizes that in life there are going to be unexpected collisions. Just as the sea creates storms life creates storms. Those who live life to the fullest will be the least affected by these storms because they have the strength and the knowledge to handle them, but the observers or those on land will be destroyed because they do not have the power to handle the destruction that the storms will cause.

Local wars had flared up in the nineteenth century, but a major war was regarded as unlikely. Looking back at 1914 today, however, we can see that each of the major countries of Europe had interests that would bring it into conflict with at least one of the other great powers. On June 28, 1914, Gacrilo Princip, a nineteen-year-old Serbian revolutionary, fired two pistols shots. One killed Archduke Franz Ferdinand, the nephew of Emperor Franz Joseph of Austria-Hungary and heir to the Austrian throne. The other killed Sophie, his wife.Austria-Hungary held Serbia responsible. On July 5 Austria asked for and recieved from Germany a "blank check" of support for any action Austria-Hungary might take against Serbia.On July 23 Austria sent a series of demands to the Serbians. The demands were designed to humiliate and virtually destroy the Serbian nation. Still, Serbia agreed to most but not all of the demands.Austria reacted on July 28 by declaring war on Serbia. The Russians prepared to defend Serbia. On July 31 the Germans sent a warning to Russia to stop mobilizing its army for war. the Russians ignored the warning, and Germany declared war on Russia on August 1. France came to the aid of its Russian ally by declaring war on Germany. The British hesi tated, but when the Germans marched into Belgium, they declared war on Germany on Aug 4.

Customers who use standards-based technology are not locked into any given vendorââ¬â¢s proprietary implementations. Microsoft supports the IETF efforts to standardize VPN technology. As time progressed many of the work place issues were addressed as well was the formation of organized labor which gave workers a voice.Despite the vast advantages the American Industrial Revolution provided society, the speed in which changes were taking place provided many opportunities to make mistakes and learn as a nation. Beneficial or Detrimental to America Overall, the events and innovatio ns that created the American Industrial Revolution were immensely beneficial to the America we know today. Although there were many opportunities to learn from the changes the country encountered, the positive impact industrialization had on society was magnificent.Improvements in transportation caused the world as a whole to become a much smaller place as travel and communication greatly improved. Work place conditions and practices eventually became regulated, fair, and safer for the American workforce. Modern American society owes a great debt to industrialization as it was one of the most transformational series of events in the nationââ¬â¢s history. Conclusion Not unlike the prehistoric discovery of fire and the wheel, American industrialization left future generations with a foundation and the tools for unprecedented innovation.The more recent revolution in technology, namely the internet and cell phone devices, owe a debt to the revolutionary period of American industriali zation. This change that started in few states had transformed the whole country from a simple agrarian importer of manufactured goods to an industrial thrust. Goloboy stated that, ââ¬Å"Interruptions in trade associated with the American Revolution encouraged industrializationâ⬠(p. xiii).Primarily, before American people realized that they could possibly become as industrialized as its mother country, England, it went through some circumstances that caused developments in economy. First, American people steadily became acquainted with buying instead of producing their basic needs; this led to the increase of the demand for those needs. Second, along with the growing demand for goods they also discovered that goods could be produced more rapidly using the technology brought by the mother country. Third, the separation of America from England after the Independence War in 1812 led to the withholding of goods from Europe, which triggered Americans to ââ¬Å"build their own factoriesâ⬠(p. xiv). By this time, many farmers left farming and moved to west to engage in factory works. It is said that industrial revolution that started in the early 19th century had caused changes in the rural system of the country that transformed the whole society.The rural system emerged while improvements in the political system took place that gave American people much opportunity to cope with changes. High standard of living continued to rise among communities while people enjoyed social, political, and economic progress as a result of American Revolution. Many jobs were opened to American women, education was improved and became compulsory to children, middle-class society also emerged, many leisure activities became part of their lifestyle, and most of all, and families achieved a better standard of living with all its comforts.Crothers in his review of Meyerââ¬â¢s ââ¬ËRoots of Industrializationââ¬â¢ emphasized that agricultural sector had fueled the industrial development in many cities; and the supply and demand grew further, which caused increase in labor force and wages and capital investment (par. 2). Local market that emerged in a given city turned to urban place where most commercial services occurred. Cities like Boston became the center of major business transaction among those cities, which according to Crothers ââ¬Å"social networks of capitalâ⬠(Meyer, 70, cited in Crothers).Basically, industrial revolution in America brought changes in the living condition of the people; primarily it transformed the rural system, which is the rise of urbanization. Urbanization came about due to the movement of more people to the cities where they got employment. Reformation follows as it brought both positive and negative impact in the life of American pe ople. Some of the positive impact of urbanization is that it brought people many benefits like finding solution to economic and social problems.For instance, education and health had improved; people finally understood the causes of many diseases and made considerable actions to that like setting of safety and health standards in homes, public places, buildings, water system, etc. Another benefit is improvement in peopleââ¬â¢s lives. Because of plenty work opportunities; they had the chances to enter the middle class. On the other hand, industrialization had weakened family life because parents who supposed to supervise the children stayed in the factory during the day; because of this, many children wandered in the streets and survived on their own.At this point of rapid industrialization, cities became prone to many social and environmental problems. The growth in population caused the dilemma of housing, sanitation, and health; crime rate got high while more and more families suffered from diseases because of pollution and stressful work in the factories. Kuser stressed that ââ¬Å"Cities became overcrowded and pollutedâ⬠(p. 7). With the influence of Romanticism ideology, American suburbs improved in the mid-1800s as transit lines, railways and urban planning were incorporated in the development.This transportation mode served as link among major cities of the United States. Conclusion Industrialization in American occurred unexpectedly perhaps, but it happened because people responded objectively to meet the demands of life. Besides, along the national independence they gained after a series of war against its mother country, agriculture grew further in many parts of the region, and manufacturing business in other region.The industrialization with influence of Great Britain transformed the rural communities until each evolved as urban.
